Our Services

Through our Home and Community-Based (HCBS) programs, PathLink helps adults with disabilities live independently in their communities by providing services tailored to their needs. The specific services we provide include:

  • Adult Day Support Offering social and recreational activities in the community.

  • Home Maker/Personal Care (HPC):  Supporting individuals in the home and the community on their path to becoming more independent; while meeting their daily living needs

  • Money Management: Assisting clients with budgeting, bill paying, banking, and financial literacy education to help them gain independence.

  • Career Planning: Working with clients to explore career options, set goals, develop resumes, practice interview skills, and find fulfilling jobs.

  • Non-Emergency Medical Transportation and Non-Medical Transportation: Providing rides to non-emergency medical appointments, physical therapy, counseling, other healthcare services, school and work.

By providing these essential services, PathLink empowers adults with disabilities to live self-sufficiently while remaining connected to their communities. Our programs are designed to meet each client's unique needs and personal goals to help them achieve an optimum level of independence and quality of life.
